Cancellation Policy

If you are unable to keep an appointment and do not call to cancel or
reschedule 24 hours prior to your scheduled appointment, you will be charged
for non-cancellations as follows:

New Patients Appointment $150.
Yearly Physical Exam $100.
Office Visit $50

**This fee is not covered by insurance.**
